Course Roster Upload Instructions

  1. Complete the form below.  Fields marked with a * are required to be completed.
  2. When attaching the Course Roster file, be sure to include the Course Roster and any associated quiz answer sheets.  It is preferred for you to upload the Course Roster and any associated answer sheets as a single file.  However, you may attach up to 10 files at a time.
  3. The Lead Instructor Email will be used to send you a copy of the form submission for your records.
