AHA Provider Card Order Form

Trinity EMS Academy American Heart Association Instructors use this page to order provider cards to be placed in their AHA  Instructor Network account.  You may order as many provider cards as you wish for the disciplines you are allowed to teach.  For example, BLS instructors may order BLS and Heartsaver provider cards, but not ACLS or PALS, etc. 

Once your order is placed, you and Trinity EMS Academy staff will receive a notification and a copy of the order form.  Trinity EMS Academy will send out an invoice for the total amount of the cards you wish to purchase.  Upon payment of the invoice, your cards will be loaded into your AHA Instructor Network account and be available for use.
